      目的 通过调查盐城市12家医疗机构儿童CT受检者不同检查部位剂量水平,研究儿童CT扫描剂量分布,为建立该地区儿童CT扫描诊断参考水平提供依据。方法 2024年在盐城市各县(市、区)采用方便抽样法,分别选取至少1家开展CT放射诊疗的医疗机构,每家医疗机构抽取2台年度状态检测合格的CT设备,调查对象分为0~1、1~、5~、10~15岁4个年龄组,按头颅、胸部、腹部扫描检查部位,调查每台设备每个年龄组20例,收集受检者基本信息、CT扫描参数、容积CT剂量指数(CTDIvol)、剂量长度乘积(DLP)等指标,分析CTDIvol、DLP等剂量指标在不同年龄组及不同级别医院间的差异。结果 4个年龄组儿童CT扫描的CTDIvol、DLP的Q3分别为:头颅:33.22、41.44、51.17、48.36 mGy和522.06、641.09、906.20、831.84 mGy·cm;胸部:3.80、3.19、4.15、6.64 mGy和71.72、67.35、112.72、229.28 mGy·cm;腹部:6.58、7.08、8.42、11.86 mGy和167.76、226.70、301.16、522.78 mGy·cm。同一部位不同年龄组之间CTDIvol和DLP差异均有统计学意义(头颅、胸部和腹部,CTDIvol:χ2=289.92、269.99、167.70,P<0.05;DLP:χ2=272.34、434.91、247.16,P<0.05)。同一部位不同医院级别之间头部和胸部CTDIvol和DLP差异有统计学意义(头部和胸部,CTDIvol:Z=-5.23、-10.05,P<0.05;DLP:Z=-5.58、-8.67,P<0.05)。结论 盐城市儿童部分年龄段头颅CT扫描剂量偏高,胸部和腹部CTDIvol和DLP的75%分位数处于偏高水平,儿童CT扫描程序还有进一步优化的必要。

      Objective To investigate the dose levels of different examination regions in pediatric patients undergoing computed tomography (CT) examinations in 12 medical institutions in Yancheng City, analyze the pediatric CT dose distribution, and provide a basis for establishing local diagnostic reference levels for pediatric CT. Methods In 2024, convenience sampling was used to select at least one medical institution performing CT diagnosis and treatment in each county (county-level city, and district) in Yancheng City. Two CT scanners that had passed the annual quality inspections were selected in each institution. Pediatric patients were divided into four age groups: 0-1, 1-, 5-, and 10-15 years. For each scanner, 20 patients per age group were surveyed for head, chest, and abdominal CT examinations. Data collected included patient demographics, CT scan parameters, volume CT dose index (CTDIvol), and dose length product (DLP). Differences in CTDIvol and DLP across age groups and hospital levels were analyzed. Results The Q3 values of CTDIvol and DLP for head CT across the four age groups were 33.22, 41.44, 51.17, and 48.36 mGy, and 522.06, 641.09, 906.20, and 831.84 mGy·cm, respectively. For chest CT, CTDIvol values were 3.80, 3.19, 4.15, and 6.64 mGy, and DLP values were 71.72, 67.35, 112.72, and 229.28 mGy·cm. For abdominal CT, CTDIvol values were 6.58, 7.08, 8.42, and 11.86 mGy, and DLP values were 167.76, 226.70, 301.16, and 522.78 mGy·cm. Statistically significant differences in CTDIvol and DLP were found among age groups for the same scan regions (head, chest, and abdomen; CTDIvol: χ2=289.92, 269.99, 167.70, P<0.05; DLP: χ2=272.34, 434.91, 247.16, P<0.05). Significant differences were also observed in head and chest CTDIvol and DLP between different hospital levels (head and chest; CTDIvol: Z=-5.23, -10.05, P<0.05; DLP: Z=-5.58, -8.67, P<0.05). Conclusions The radiation doses of head CT scans for certain age groups of children in Yancheng City are relatively high, and the Q3 values of CTDIvol and DLP for chest and abdomen are at relatively high levels. It is necessary to further optimize pediatric CT scanning protocols.
