      癌症目前仍然是导致人类死亡的第二大原因。放射治疗是恶性肿瘤的常用治疗方式,在有效提升肿瘤控制效果的同时,也会对周围正常组织造成不同程度的损伤。盆腹腔恶性肿瘤患者接受放疗后,80%患者出现不同程度的急/慢性胃肠道反应,反应严重时不仅会被迫中断放疗,还会提升肿瘤复发的可能性,甚至影响患者远期生活质量。因此,放射性肠炎(radiation enteritis, RE)已成为限制放疗疗效与安全性的重要因素。近年来,分子放射生物学与肠道微生态领域的研究不断深入,RE的发病机制逐渐明晰,其发生涉及肠上皮屏障受损、机体免疫稳态失衡、活性氧异常蓄积以及肠道菌群结构紊乱等多个关键环节。目前,针对RE的防治策略在不断发展,但仍然缺乏一致共识。本文旨在就RE的发病机制及其综合防治策略的研究进展作一综述,为提升盆腹腔肿瘤放疗的临床安全性、改善放疗患者的生活质量提供相应的理论依据和实践思路。
英文摘要:
      Cancer remains the second leading cause of human death at present. Radiotherapy, a common treatment method for malignant tumors, tends to cause varying degrees of damage to the surrounding normal tissues while effectively controlling tumors. After radiotherapy, 80% of patients with pelvic and abdominal malignancies experience acute/chronic gastrointestinal complications. In severe cases, these complications lead to radiotherapy interruption, elevate the possibility of tumor recurrence, and even affect the long-term quality of life of the patients. Therefore, radiation enteritis (RE) has emerged as an important factor limiting the efficacy and safety of radiotherapy. In recent years, an increasing number of insights into molecular radiobiology and intestinal microecology have shed light on the pathogenesis of RE. It has been revealed that the occurrence of RE involves several critical processes: damage to the intestinal epithelial barrier, imbalanced immune homeostasis, dysregulated accumulation of reactive oxygen species, and disordered intestinal flora structure. Currently, despite the continuous evolution of prevention and treatment strategies for RE, there remains a lack of consensus on them. This study reviews the advances in research on the pathogenesis of RE and corresponding comprehensive prevention and treatment strategies. The purpose is to provide a theoretical basis and practical ideas for improving both the clinical safety of radiotherapy for pelvic and abdominal tumors and the quality of life of the patients.
