      脑转移瘤是晚期非小细胞肺癌(NSCLC)患者最严重的致死性并发症,其中驱动基因阳性的NSCLC患者更易发生脑转移,尤其是表皮生长因子(EGFR)基因突变患者的脑转移发生率更高,其发生发展严重影响患者的生活质量及预后。随着分子靶向药物的出现,针对NSCLC脑转移瘤的治疗方式更加个体化和精准化。同时,立体定向放射外科(stereotactic radiosurgery, SRS)已经成为NSCLC脑转移瘤最重要的局部治疗手段,与靶向治疗的联合模式是当今的研究热点,二者联合具有协同作用,可以改善驱动基因阳性的NSCLC脑转移患者的预后。但是,目前对于无症状脑转移瘤患者,SRS的介入时机尚存在争议。因此,本文就近年来SRS联合靶向治疗驱动基因阳性NSCLC脑转移的策略及相关研究进展等方面进行文献报道综述。

      For patients with advanced non-small cell lung cancer (NSCLC), brain metastases (BM) represent the complication with the highest lethality. Patients with driver gene-positive NSCLC, especially those with epidermal growth factor receptor (EGFR) gene mutations, are more prone to suffer from BM. The occurrence and progression of BM seriously affect the quality of life and prognosis of these patients. The emergence of molecular targeted drugs contributes to more individualized and precise treatment for BM as a complication of NSCLC. Meanwhile, stereotactic radiosurgery (SRS) has become the most important therapy for the local treatment of BM from NSCLC. The combination of targeted therapy and SRS has become a hot research topic since the synergistic effects of both can improve the prognosis of driver gene-positive NSCLC patients with BM. However, for patients with asymptomatic BM, the optimal intervention timing of SRS remains controversial. This literature review presents the strategy and relevant research progress of SRS combined with targeted therapy for BM from driver gene-positive NSCLC in recent years.
