      目的 比较宫颈癌后装放疗中使用不锈钢曼彻斯特施源器时TG43算法与AcurosBV算法的剂量学差异,为临床精准剂量计算提供依据。方法 回顾性分析四川省人民医院21例使用不锈钢曼彻斯特施源器的宫颈癌患者,在瓦里安Eclipse计划系统中分别采用TG43算法和AcurosBV算法计算剂量,统计高危临床靶区(HRCTV)D90%D98%及膀胱、直肠、小肠、尿道等危及器官(OAR)D2 cm3D0.1 cm3剂量差异。开发基于Python的高密度体素自动识别工具,提升AcurosBV算法的计算成功率。结果 AcurosBV算法的HRCTV剂量显著低于TG43算法,D90%降低了7.46%(t=91.88,P<0.01),D98%降低了7.27%(t=39.47,P<0.01);危及器官剂量降幅为5.53%~8.80%(P<0.01),其中小肠D2 cm3差异最大(-8.80%)。Python工具可有效解决临床漏检问题。结论 AcurosBV算法在不锈钢施源器中表现出显著的剂量修正效应与更优的计算精度。本研究结果为TG43算法向模型化剂量计算算法(MBDCA)的临床转化提供了必要的剂量学参考依据。
英文摘要:
      Objective To evaluate dosimetric differences between TG43 and AcurosBV algorithms for stainless-steel Manchester applicators in cervical cancer brachytherapy, and to provide a basis for precise clinical dose calculation.Methods A retrospective study analyzed 21 patients with cervical cancer who were treated with stainless-steel Manchester applicators at Sichuan Provincial People’s Hospital. Doses were calculated using TG43 and AcurosBV algorithms in Varian Eclipse, with comparisons of D90% and D98% of high-risk clinical target volume (HRCTV) as well as D2 cm3 and D0.1 cm3 of (P<0.01) organs at risk (OARs), including the bladder, rectum, small intestine, and urethra. A Python-based automated tool for high-density voxel identification was developed to improve the computational success rate of the AcurosBV algorithm. Results AcurosBV yielded significantly lower doses than TG43 for HRCTV, with 7.46% and 7.27% reductions for D90% and D98%, respectively(t=91.88,39.47,P<0.01); the decreases in doses for OARs ranged from 5.53% to 8.80%(P<0.01), with the largest difference in small intestine D2 cm3(-8.80%). The Python tool effectively addressed clinical under-detection. Conclusions For stainless-steel applicators, the AcurosBV algorithm demonstrated a significant dose correction effect and superior calculation accuracy. These results provide an essential dosimetric reference for the clinical transition from TG43 to model-based dose calculation algorithms.
