| 目的 评估接受胸部非增强CT检查的儿童患者光子计数CT和能量积分探测器CT的辐射剂量、客观和主观图像质量的差异。方法 回顾性收集2024年4月至2025年5月在郑州大学第一附属医院行光子计数探测器CT (PCD-CT)胸平扫的儿童患者52例,同时,回顾性收集2023年10月至2025年5月在郑州大学第一附属医院行传统能量积分探测器CT (EID-CT)胸平扫的儿童52例。在图像客观评价中,计算双肺上叶及下叶肺组织的信噪比(SNR)、对比噪声比(CNR)及背部肌肉噪声(SD);在图像的主观评价中,5分法对各组图像的总体图像质量,肺部结构及软组织显示能力,运动伪影进行评分。两组间SNR和CNR的比较采用独立样本t检验或非参数检验,主观评分比较采用Mann-Whitney U检验。结果 在客观评价中,对于SNR肺组织、CNR肺组织及SNR肌肉,EID-CT图像均低于PCD-CT图像,差异均有统计学意义(Z=7.04~7.66、7.05~7.68,t=6.61~17.38,P<0.05)。在6~9岁组和10~12岁组中,PCD-CT的剂量-长度乘积(DLP)与E均低于传统EID-CT (t=2.99、0.16,P<0.05)。在主观评价中,PCD-CT图像质量主观评分均高于EID-CT,差异有统计学意义(Z=3.56~4.06,P<0.001)。结论 PCD-CT在客观和主观图像质量、降噪、空间分辨率及组织对比度方面均优于EID-CT。 |

| Objective To evaluate the differences in radiation dose, objectiveimage quality, and subjective image quality between photon-counting detector CT (PCD-CT) and conventional energy-integrating detector CT (EID-CT) when children underwent non-enhanced chest CT. Methods A total of 52 children who underwent non-contrast chest CT scans using photon-counting detector CT (PCD-CT) at the First Affiliated Hospital of Zhengzhou University between April 2024 and May 2025 were retrospectively enrolled. Otherwise 52 children who underwent non-contrast chest CT scans using energy-integrating detector CT (EID-CT) at the same institution between October 2023 and May 2025 also were retrospectively selected. Two radiologists subjectively evaluated image quality using a 5-point Likert scale and measured the CT values and noise levels in the upper lobes of both lungs, the lower lobes of both lungs in the areas of no obvious pulmonary markings, as well as in the aorta and back muscles at the same layer. The optimized signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), and radiation dose were calculated. Comparisons of SNR and CNR between the two groups were performed using independent samples t-tests or non-parametric tests, while subjective scores were compared using the Mann-Whitney U test. Results In the objective evaluations, the SNR and CNR of the lung on EID-CT images were lower than that on PCD-CT images, which showed statistically significant differences (Z=7.04-7.66, 7.05-7.68, t=6.61-17.38, P<0.05); the effective radiation dose (E) of PCD-CT were lower than traditional EID-CT in 6-9 and 7-12 groups, and showed statistically significant differences (t=2.99, 0.16, P<0.05). In the subjective evaluations, compared to the EID-CT images overall image quality scores were higher for the PCD-CT images, and the differences were statistically significant (Z=3.56-4.06,P<0.001). Conclusions Compared to EID-CT images the PCD-CT images have higher spatial resolution, tissue contrast and lower image noise. |
