      目的 评估本机构近3年常见CT检查的辐射剂量水平,通过建立机构CT辐射剂量诊断参考水平(DRL),监测和优化放射防护实践,为实现CT检查辐射剂量的精准控制与系统性优化提供依据。方法 回顾性收集本机构2022年至2024年进行头颅、颈部、胸部、腹部、盆腔、腰椎平扫,以及冠状动脉(前瞻)、颅脑、颈部、胸腹部CT血管成像(CTA)检查的剂量长度乘积(DLP)。统计辐射剂量分布的第25%(P25)、50%(中位数,P50)及75%(P75)位数,以P75作为本机构的DRL用于后续分析。参照《WS/T637-2018》标准,对本机构各部位剂量中位数及DRL值进行评估,并与北京市2023年DRL数据进行对比,分析本机构各部位近3年DRL值的动态演变趋势。结果 与北京市2023年DRL相比,本机构大部分检查部位的DRL值偏低或基本持平,仅个别部位(如头颅)略高于北京市水平,但整体仍处于合理可控范围。本机构10个部位的DRL值均显著低于WS/T637-2018标准的诊断参考水平。纵向比较发现,近3年间各部位的辐射剂量差异统计学有意义(P<0.05)。结论 本机构的CT检查辐射剂量控制有效,所有受检部位的诊断参考水平均优于卫生行业标准,体现了良好的辐射防护质量管理。
英文摘要:
      Objective To assess the radiation dose levels of routine CT examinations in the Department of Radiology of our institution over the past three years, with the aim of establishing local diagnostic reference levels (DRLs) to monitor and optimize radiological protection practices and ensure patient safety. Methods A retrospective collection of dose-length product (DLP) data was performed for ten common examination types conducted from 2022 to 2024: non-contrast axial head, neck, high-resolution chest (HRCT), abdomen, pelvis, lumbar spine, prospective coronary Computed Tomography Angiography CTA, cranial CTA, neck CTA, and thoracoabdominal CTA. The 25th (P25), 50th (median,P50), and 75th (P75) percentile values were calculated, with the P75 value designated as the local DRL for subsequent analysis. These local DRLs were compared against the national standard for adult DRLs published by the National Health Commission of China in 2018 (WS/T637-2018). Furthermore, the trend in DRL values for each examination type over the three-year period was analyzed. Results This study included a total of 936,263 examinations. The results demonstrated that the local DRLs (P75) for DLP across all ten examined areas were significantly lower than the corresponding values in the WS/T637-2018 national standard. Compared with the 2023 Beijing DRL data, the DRL values for most examination sites at this institution were lower or roughly comparable, with only a few sites (e.g., head) being slightly higher than the Beijing levels. Overall, however, the radiation doses remained within a reasonable and controllable range. Additionally The differences in radiation doses among various anatomical sites over the past three years were statistically significant.Conclusions The radiation dose control for CT examinations in our institution is effective. The diagnostic reference levels for all examined areas are superior to the national standard, reflecting high-quality management of radiation protection.
