| 目的 探讨用组织块培养图像分析技术测定鼻咽肿瘤组织SF2值对肿瘤放射效应的预测价值。方法 用组织块培养图像分析技术测定46例鼻咽癌患者新鲜的鼻咽部活检病理标本的SF2,并用CT和临床体检手段评估患者放疗过程中的鼻咽部肿瘤消退情况和颈部转移淋巴结消退情况,以放疗结束后3月时CT和临床资料评估即时疗效。结果 46例活检肿瘤组织的SF2与鼻咽部肿瘤完全消退剂量、转移淋巴结全消退剂量、40Gy时鼻咽部肿瘤消退比和40Gy时淋巴结消退比之间有明显的相关性,r值分别为0.881(P<0.01)、0.751(P<0.01)、-0.733(P<0.01)、-0.609(P<0.01)。按SF2的中位数值分组,SF2高组和低组之间的上述观察指标之间差异均有非常显著性(P<0.01);同时SF2值低组患者的即时疗效明显优于SF2高组。结论 鼻咽肿瘤组织的SF2值在一定程度上可以反映患者肿瘤的放射效应,其值越小肿瘤消退越快,肿瘤放疗的近期局控率越高。 |

| Objective To explore the prediction value of radiation effect on survival fraction at two grey(SF2) for bioptic tissue from nasopharyngeal carcinoma. Methods With fresh bioptic specimens from 46 cases of nasopharyngeal carcinoma in Out-patient Department of Tumor Center of Sun Yat-sen University,the survival fraction at 2 gray (SF2) was estimated by the method of image analysis of tissue fragment primary culture.Complete remission doses of nasopharyngeal tumor (CRD-NT),complete remission doses of metastatic cervical lymph nodes (CRD-LN),nasopharyngeal tumor regression rate at 40 gray (NTRR-40),lymph nodal regression rate at 40 Gy (LNRR-40) were assessed by computer tomography scanning and clinical physical examination.The immediate curative effect (ICE) in the patients was evaluated by physical examination and the results of CT scanning at the third month after radiotherapy. Results There were obvious correlations between SF2 and CRD-NT,NTRR-40,CRD-LN,LNRR-40.The partial correlation coefficients were 0.881 (P<0.01),-0.733(P<0.01),0.751((P<)0.01) and -0.609 (P<0.01),respectively.All the 46 cases were grouped by the median of SF2 values.Significant statistical differences of NTRR-40 (P<0.01),CRD-NT(P<0.01),LNRR-40(P<0.01),CRD-NL((P<)0.01) and ICE (P<0.01) were found between the group of SF2 values above the median and the group of SF2 values below the median.Significant statistical difference in CRD-NL (P<0.05) was found in the group of N stage. Conclusion This investigation showed that regression rate has a decreasing trend obviously along with increasing SF2 values.There is a significient correlation between the SF2 value and the immediate curative effect.These results cue that SF2 values could reflect radiation effect on tumor tissue in NPC patients to a certain extent. |
