      目的 探讨早期放射性肺纤维化的始动部位及其变化规律。方法 大鼠右胸部行单次照射,吸收剂量分别为0,15,30Gy,照后又分为1,3,5月组。用生化法检测肺羟脯氨酸;用偏振光法检测Ⅰ、Ⅲ型胶原;用免疫组化法检测血管紧张素Ⅱ(AⅡ)分布。结果 肺纤维化程度与照射剂量和照后时间有关;Ⅰ、Ⅲ型胶原比例随纤维化程度加大而增加;增生的胶原纤维起自细小支气管和细小动脉外膜的成纤维细胞,肺泡间隔中Ⅰ型胶原取代Ⅲ型胶原;间质成纤维细胞和巨噬细胞AⅡ免疫组化阳性。结论 照射可刺激肺间质成纤维细胞增生,Ⅰ型胶原取代Ⅲ型胶原;间质细胞中AⅡ的表达与合成可能对此过程有一定的促进作用。

      Objective Production site development and alterations of early pulmonay fibrosis were studied. Method Single irradiation was made at right thorax of rats with 0,15 and 30 Gy of γ irradiation, respectively.The rats were divided into three groups which were sacrificed 1,3,5 months post irradiation.Hydroxyproline in lungs was measured by biochemical method.Pulmonary type I and Ⅲ collagens were measured by polarization method.Distribution of angiotensin Ⅱ (AⅡ) in pulmonary tissues was displayed by immunohistochemical method. Results Extent of pulmonary fibrosis relatively increased with irradiation dose and time elapse after irradiation.Ratio of type I to type Ⅲ collagens increased with increasing fibrosis.Proliferating collagen fibers mainly came from fibroblasts of pulmonary bronchial and arteriolar adventitia,and extended into pulmonary parenchyma.Meanwhile,type I collagen substituted for type Ⅲ collagen in interstitium of pulmonary alveoli.AⅡ was positive for fibroblasts and macrophages in pulmonary interstitium. Conclusion Irradiation can stimulate fibroblasts in interstitum proliferation,and type I collagen substitutes for type Ⅲ collagen.Expression and synthesis of AⅡ in interstitium may promote the course of pulmonary fibrosis.
