水凝胶在前列腺癌和宫颈癌放疗中对直肠的保护

[摘要] 放疗是前列腺癌和宫颈癌的重要治疗方法,但是有效治疗肿瘤的同时会造成周围危及器官损伤,以直肠损伤最为常见。有报道在前列腺癌和宫颈癌放疗中,将水凝胶注射在前列腺与直肠、阴道与直肠之间增加间隙距离,可以降低直肠照射剂量,减少发生放射性损伤的风险。本文就水凝胶在前列腺癌和宫颈癌放疗中的应用、对直肠等危及器官的保护以及生活质量的提高等进行综述。

Protection of hydrogel for rectum in radiotherapy of prostate cancer and cervical cancer

[Abstract] Radiotherapy is an important treatment for prostate cancer and cervical cancer. However, it can cause damage to the surrounding organs while effectively treating the tumor. Rectal injury is the most common site. It has been reported that in the radiotherapy of prostate cancer or cervical cancer, hydrogels are injected between prostate and rectum or vagina and rectum to increase the gap distance, which can reduce rectal radiation dose and the risk of radiation damage. This article reviews the applications of hydrogels in the treatment of prostate cancer and cervical cancer, to protect the rectum for better quality of life.

前列腺癌和宫颈癌放疗时,由于直肠与前列腺和阴道靠近,剂量无法快速下降[1],增加发生放射性直肠炎风险,据报道晚期2级以上肠道不良反应分别为13.7%和17.0%[2-3],严重影响患者生活质量。在前列腺与直肠、阴道与直肠之间注射水凝胶,不仅可以有效提高靶区剂量,还可以降低直肠等危及器官照射剂量,从而减少放射性损伤发生率,提高患者生活质量。

一、水凝胶在前列腺癌放疗中的应用

1.概述:目前,用于增加前列腺与直肠间距的主要是聚乙二醇水凝胶(polyethylene glycol hydrogel,PEG)[4],PEG具有耐受性好、操作简便、成功率高、并发症少等优势。

在CT或直肠超声引导下,将水凝胶注射在前列腺与直肠之间的脂肪组织和疏松结缔组织,水凝胶10 s内凝固并形成可吸收的水凝胶间隔,稳定存在3个月,6个月后液化、吸收、后经肾脏过滤清除[5],9~12个月可完全吸收[6]。将水凝胶注射在前列腺与直肠间的具体操作过程见文献[7]。研究证实,在前列腺癌患者中植入水凝胶的成功率超过97%[5, 8-9]。植入水凝胶后并发症主要为感染,Wu等[9]发现1例患者(1/18)会阴部出现脓肿,后行切开引流和抗感染治疗后好转。据报道,预防性使用抗生素可将感染风险降低到5%以下[10]。一组患者(50例)术前预防性使用环丙沙星,其中3位出现不同程度感染,另一组患者(50例)调整抗生素为头孢曲松和庆大霉素,没有患者出现感染[8]。在一项Ⅲ期临床研究中,植入水凝胶后6.7%的患者出现短暂的1级不良事件(轻度血精、肛门直肠压迫、血尿、坐姿不适等),3.3%的患者出现需要药物治疗的2级不良事件(轻度下尿路症状、低血压和中度肛周疼痛),但是他们认为这与水凝胶本身无相关性[11]。Heikkil等[12]研究结果显示,植入水凝胶后1例(10%)出现直肠压迫,1例(10%)出现突然排便,均无需干预。最近,在MAUDE数据库中发现植入水凝胶后也发生过急性肺栓塞、过敏反应、败血症和直肠尿道瘘等不良反应[13-14],但是这些并发症发生率低,并且是否与植入水凝胶有关仍存在争议。

2.水凝胶对于直肠等危及器官的保护

(1) 在前列腺癌外照射中的应用:水凝胶植入后,不同外照射技术都可以实现直肠剂量的下降。最早Susil等[15]在尸体上进行试验,植入水凝胶后前列腺与直肠间距增加到12.5 mm,直肠V70相对降低了79.9%。随着放疗技术不断发展,为了探讨水凝胶在不同外照射技术下的效果,Pinkawa等[16]对18例前列腺癌患者植入PEG,分别使用三维适形放疗(3D-CRT)和调强放疗(IMRT)制定放疗计划,直肠V70分别由14.4%降至6.1%(相对下降56%)、由17.2%降至7.5%(相对下降58%),结果证实无论使用何种外照射技术,水凝胶都可以降低直肠剂量。Weber等[17]也证实,在植入水凝胶前后分别使用IMRT、容积旋转调强放疗(VMAT)和调强质子治疗(IMPT)制定计划,计算植入前后直肠V70,IMRT为9.8%和5.3%,VMAT为10.1%和3.9%,IMPT为9.7%和5.0%。

对于水凝胶植入体积与直肠前列腺间距、直肠剂量的关系,有研究表明,当前列腺与直肠间距增加到15 mm时,直肠剂量降至最小[15]。Pinkawa等[18]把64例前列腺癌患者随机分为两组,第1组前列腺与直肠间距平均为11 mm,直肠V70为6.0%,相对减少62.5%;而第2组间距平均为15 mm,直肠V70为2.0%,相对减少80%。但是,目前尚未有研究证明进一步分离前列腺与直肠会有额外临床效果。

水凝胶间隔可以显著降低直肠剂量,但是对于膀胱、阴茎海绵体的保护论据不足。Song等[19]对52例前列腺癌患者植入10 ml PEG,植入前后膀胱V70分别为14%和12.2%(P=0.24),差异无统计学意义。Hamstra等[20]研究显示使用水凝胶间隔后,阴茎海绵体平均剂量由21 Gy下降到11 Gy、最大剂量由46 Gy下降到36 Gy(P<0.05)。Karsh等[11]将患者随机分为对照组(73例)和试验组(149例),阴茎海绵体照射剂量分别为21.1和10.8 Gy(相对减少49%,P=0.036),膀胱剂量则无明显差异,结果表明水凝胶对阴茎海绵体存在保护效果,而对膀胱效果不佳。

(2) 在前列腺癌近距离治疗中的应用:近距离治疗(brachytherapy,BT)具有独特的剂量学优势,随着距离的增加剂量快速衰减,实现肿瘤高剂量的基础上降低周围危及器官的损伤,然而植入水凝胶后仍可降低直肠放射性损伤[21]。Wu等[9]为了评估水凝胶在高剂量率近距离治疗(HDR-BT)中的剂量学效果,对18例植入水凝胶患者和36例未植入水凝胶患者进行研究,结果表明,试验组与对照组直肠V70分别为4.0%和46%(P<0.0005),直肠剂量显著降低。同样,Kahn等[22]对80位接受低剂量率近距离治疗(LDR-BT)患者进行研究,其中40例植入水凝胶,发现水凝胶组直肠D1 cm2D2 cm2D5 cm2分别减少了32%(P<0.01),26%(P<0.01)和17%(P<0.01)。

对于中高危患者,因经常使用外照射联合近距离治疗,进一步增加了直肠放射性损伤的风险。Yeh等[23]研究显示,接受HDR-BT+IMRT的患者植入水凝胶后直肠D2 cm2由55.2%减少到47.9%, 直肠平均剂量和直肠最大剂量相对减少了7%、17%。同样,Chao等[24]报道了97例接受HDR-BT+IMRT的患者,其中32例植入水凝胶,结果表明直肠V30V40V50V60V75V80分别降低了36.4%、58.1%、76.7%、95.2%、100%、100%、100%。

3.增加前列腺与直肠间距对放射性损伤和生活质量的影响:植入水凝胶可以增加前列腺与直肠距离,从而降低放射性损伤的发生率,提高患者生活质量。放射性损伤分为急性期和晚期不良反应。对于急性期肠道、尿道不良反应,Mariados等[25]研究表明,间隔组与对照组之间差异无统计学意义(急性期肠道不良反应分别为23%、27.8%,P=0.525;尿道不良反应分别为45.8%、52.7%,P=0.488),但是在急性直肠疼痛方面,间隔组明显优于对照组(2.7%、11.1%,P=0.022),之后Krash等[11]的研究也得出同样的结果。对于晚期肠道不良反应,Hamstra等[26]研究显示间隔组和对照组1级、2级肠道不良反应分别为2%、9%(P<0.03);0、6%(P<0.015),差异具有统计学意义,同样Te Velde等[27]也证实间隔组患者1、2级晚期直肠炎发生率是9.2%,明显低于对照组的26.7%(P=0.015)。然而对于晚期尿道不良反应,多项研究表明植入水凝胶并无明显改善[1125-26]

生活质量主要表现为肠道、尿道、性功能3方面,一项Ⅲ期临床研究使用EPIC(Expanded Prostate Cancer Index Composite)对患者生活质量进行评估,治疗结果后36个月间隔组和对照组在肠道、尿道生活质量评分分别为+0.5、-5.3和+0.6、-3.3,差值分别为5.8(P=0.002),3.9(P=0.04),结果表明,植入水凝胶可以显著提高患者晚期生活质量[26]。在性功能方面,文献[11]报道治疗后37个月,间隔组和对照组患者阴茎恢复功能的比例分别为66.7%和37.5%(P=0.046),显著提高了性生活质量。

二、水凝胶在宫颈癌放疗中的应用

1.概述:宫颈癌放疗通常需要外照射与后装治疗结合,目前在宫颈癌后装治疗中,降低直肠剂量的方法包括纱布填塞、球囊填塞。传统方法纱布填塞耗时长,而且填塞和移除纱布时常给患者带来痛苦[28]。球囊填塞技术提高了填塞的效率和舒适度,但是高成本以及适形性差导致无法广泛应用[29]。近几年在阴道与直肠间植入水凝胶为宫颈癌患者提供了一种新的方法。

目前,临床上用于增加阴道与直肠距离的水凝胶主要为TraceIT水凝胶,TraceIT水凝胶是一种结构稳定的PEG,具有低免疫原性、低毒性、不良反应少、良好的CT和MRI成像等特性[28-29],并且注射过程不受时间限制[30]。Bair等[31]在妇科肿瘤后装治疗中用TraceIT水凝胶作为基准标记物,结果证实水凝胶耐受性好,易于观察,无不良反应。但是有关宫颈癌植入水凝胶的研究大部分是在尸体上进行的,因此关于术后并发症报道较少。Peach等[28]研究指出,植入水凝胶后,腔内温度平均升高5℃,通过对热痛分析,这并不会造成患者不适[32]。在Ahmed等[30]的研究中,患者植入水凝胶后出现轻度恶心、呕吐、腹泻,考虑与术前服用甲硝唑、左氧氟沙星有关。

2.水凝胶对于直肠等危及器官的保护:宫颈癌患者植入水凝胶主要为局部复发后接受后装治疗,直肠保护效果明显。最早Marnitz等[33]证明了水凝胶在宫颈癌患者放疗中的可行性以及安全性,但是发现仅子宫颈腹膜外侧直肠发生移位,他们认为这与直肠子宫凹陷程度以及直肠阴道隔内疏松组织有关。之后Viswanathan等[34]对3位局部复发的宫颈癌患者进行临床研究,在阴道残端与直肠间隙植入聚合物水凝胶,间距平均增加到10 mm,直肠剂量减少了11%。2019年Ahmed等[30]报道了2例宫颈癌局部复发的患者,在行后装治疗前植入TraceIT水凝胶,植入水凝胶后阴道残端与直肠距离增加到16和20 mm,直肠D2 cm2分别由植入前5.26 Gy降至1.16 Gy,7.72 Gy降至4.99 Gy,分别减少了78%、35%。

此外,为了评估纱布填塞、TraceIT水凝胶以及纱布填塞联合TraceIT水凝胶对于直肠等危及器官保护效果,Damato等[35]在女性尸体上进行试验研究,结果显示纱布填塞联合水凝胶组直肠D2 cm2减少22%(P=0.02),膀胱D2 cm2减少10%(P=0.27),乙状结肠D2 cm2无变化,仅水凝胶或仅纱布填塞组直肠、膀胱和乙状结肠D2 cm2减少差异均无统计学意义(P>0.05),因此认为纱布填塞联合TraceIT水凝胶是最优选项。

3.增加阴道(残端)与直肠间距对放射性损伤和生活质量的影响:宫颈癌局部复发的患者植入水凝胶可以有效分离阴道残端与直肠,降低肠道和泌尿道放射性损伤,提高患者生活质量。Ahmed等[30]对患者行放射不良反应评估(common terminology criteria for adverse effects v 4.0),治疗期间1例患者出现1级放射性膀胱炎、放射性直肠炎,治疗结束后随访未发生任何放射性肠道和泌尿生殖道不良反应。同样,Marnitz等[33]和Viswanathan等[34]研究显示,随访患者均未发生急性及晚期肠道不良反应。由于宫颈癌患者植入水凝胶研究较少,无法对放射性不良反应和晚期生活质量进行充分评估,在未来还需要一些前瞻性研究进行验证。

三、总结

综上所述,前列腺癌无论接受外照射还是近距离治疗,植入水凝胶后直肠与前列腺间隙可以增加到7~15 mm,直肠V70降至5%左右,远低于标准限量25%,显著降低放射性直肠炎的发生率,提高患者肠道、尿道、性功能生活质量。对于局部复发宫颈癌后装治疗,植入水凝胶后阴道残端与直肠阴道隔距离增加到10~20 mm,直肠D2 cm2降低至5 Gy以下,可以改善肠道及尿道生活质量。

虽然目前有关水凝胶的临床研究较多,但很少应用于实际临床工作,分析原因如下:①对于植入水凝胶后出现的并发症存在争议[36],目前文献报道的并发症较轻,但是最近在MAUDE数据库[13-14]报道了一些罕见且严重并发症,需要进一步证实是否与植入水凝胶有关。②大部分文献随访时间仅3年,缺乏对于安全性、有效性和生活质量的远期评估[36-37]。③对于前列腺癌高危患者,植入水凝胶的可行性和能否提高治愈率需要进一步验证[37]。④有些学者认为,因为放疗技术(如IMRT,BT,图像引导)的提高,治疗前列腺癌患者引发的肠道不良反应在可接受范围之内[38]。⑤因为女性直肠子宫凹陷和腹腔内外组织较男性更为疏松,注射水凝胶无法使直肠与宫颈充分分离[33],这导致宫颈癌患者应用水凝胶的研究较少,缺乏足够的临床证据。⑥SpaceOARTM是获得美国食品和药物管理局认可的唯一水凝胶产品[6],但是目前没有在国内上市,这也制约了水凝胶在国内应用。因此,今后仍需大量前瞻性研究进行佐证。
