拓扑异构酶(topoisomerase enzymes,Topo)是20世纪70年代发现的一种能催化DNA拓扑异构体相互变换,调节DNA三维结构的核酶。Topo是DNA复制、转录过程必需的,其作用是使DNA超螺旋结构解链,以及在DNA复制中通过形成磷酸二酯键,介导不稳定的DNA单链瞬时断裂和再连接[1]。Topo按作用机制分为TopoⅠ和TopoⅡ两类,TopoⅠ在DNA复制延长阶段可以代替TopoⅡ,TopoⅡ在快速生长细胞中活性高,且有细胞周期依赖性,介导DNA双链的瞬时断裂和再连接,其在真核细胞中分离多级连锁子染色体的作用是不可替代的[2]。在应用中发现部分拓扑异构酶抑制剂具有放射增敏作用,本文就拓扑异构酶抑制剂放射增敏相关研究进展进行综述。
一、TopoⅠ抑制剂多项基础及临床研究发现,TopoⅠ抑制剂具有放射增敏作用[3-6],其增敏作用机制并非直接的细胞毒作用,可能涉及以下多方面[7-9]:①TopoⅠ抑制剂特异性作用于S期,使细胞周期停滞在G2/M期[6, 10-11]。②TopoⅠ抑制剂共价连接于DNA上,稳定TopoⅠ-DNA断裂复合体,妨碍TopoⅠ介导的单链DNA切口的再连接,抑制断裂的单链DNA修复,与DNA复制叉冲突,导致DNA双链断裂,增加致死性损伤[2, 8, 11]。③影响DNA转录过程中RNA合成,导致转录提前终止[12]。④作用于细胞周期检查点导致细胞凋亡[13]。TopoⅠ抑制剂主要是喜树碱(camptothecin,CPT)类药物,CPT为20世纪70年代提取自一种中国的树木喜树所产生的喹啉类生物碱,[14]是第一个被研究的拓扑异构酶抑制剂。虽然其具有抗肿瘤作用,但在实际应用中因为不良反应太大,无法直接应用于临床[15],同样受毒性限制的还有喜树碱的天然类似物羟基喜树碱(hydroxycamptothecin,HCPT)。临床应用的TopoⅠ抑制剂大都是在CPT基础上改构合成的,目前应用于临床的主要是拓扑替康(topotecan,TPT)和伊立替康(irinotecan, CPT-11),其他新一代临床候选药物包括贝洛替康(belotecan,CKD-602)、吉马替康(gimatecan, BNP-1350)、勒托替康(iurtotecan)、希拉替康(silatecan, DB-67)[16],以及处于临床前试验的、未正式命名的CPT化合物CPT417[1]、ST1968[17]、ST1976[18]、BMS-286309[19]等。
1. TPT:是半合成、水溶性的天然喜树碱类衍生物,已广泛应用于多种实体肿瘤如小细胞肺癌、非小细胞肺癌(NSCLC)、卵巢癌、结肠癌、脑胶质母细胞瘤的治疗,并取得了一定疗效[20-21]。TPT联合外照射在动物实验中显示出更强的细胞杀伤作用。Zhang等[6]在研究TPT联合放射治疗鼻咽癌小鼠模型实验中发现,能明显抑制肿瘤生长,延长无瘤生存期,有明显的增益效应。但TPT联合放化疗在Ⅰ、Ⅱ期临床试验中并不如动物实验的协同效果明显。Seung和Ross[22]进行Ⅱ期临床试验的研究显示,20例局部晚期非小细胞肺癌患者接受TPT同步放化疗加巩固化疗,3年生存率为32%,中位生存18个月。国内相似的Ⅱ期临床研究中,35例局部晚期非小细胞肺癌患者,3年生存率21.2%,中位生存期为17个月[23],两项研究TPT同步放化疗,给药方法不同但结果类似,显示晚期NSCLC放疗联合TPT治疗具有临床有效性和安全性,长期随访有较好的疗效,同样在局部晚期小细胞肺癌患者中也取得相似效果[24],但与铂类为主的放化疗方案差异不大。Lesimple等[25]进行的TPT联合放疗对无法切除的脑胶质母细胞瘤Ⅱ期临床研究显示,TPT具有渗透血脑屏障并抑制脑胶质母细胞瘤生长的能力。50例患者中,1/3治疗有效或病灶稳定,1年生存率为42%,中位生存40个月,但对患者的生存获益未见明显增加。TPT的不良作用主要是骨髓抑制,最主要是中性粒细胞减少,其不良作用一定程度上限制了临床应用,有部分学者认为TPT和CPT-11在临床剂量范围内不良反应显著,仅适用于复发或转移肿瘤[18, 26]。
2. CPT-11:也是半合成、水溶性的天然喜树碱类衍生物,Illum[27]在CPT-11联合放化疗治疗直肠癌的研究中认为,CPT-11在临床前研究及联合新辅助放化疗早期临床试验中显示出较好的放射增敏效果,然而在一个较大的随机Ⅱ期临床试验中,与以5-FU为基础的放化疗临床疗效比较,CPT-11并没有显示出更明显的增强或协同作用[28]。可能的原因是不同患者的肿瘤的Topo Ⅰ表达量有差异;其次是CPT-11转化为活性产物7-乙基-10-羟基喜树碱(SN-38)的转化率[27, 29-31]。Goldenberg等[32]将SN-38共价结合到一种人源抗-TROP-2抗体hRS7上,形成抗Trop-2 hRS7-CL2A-SN-38抗体药物共轭体(antibody-drug conjugate,ADC),在动物实验中,显示其对上皮源性肿瘤有显著和特异的抗肿瘤作用。Ⅰ期临床试验显示,这类ADC对难治性转移性实体癌具有一定效果,后续的临床研究仍在进行中。抗体药物共轭形式的放射增敏效果有待进一步研究。CPT-11常用剂量的不良反应包括:腹泻、中性粒细胞减少、脱水、疲劳、恶心、呕吐。最常见的3~4级不良反应是腹泻和中性粒细胞减少,大约发生在20%~30%的患者[33]。
3. HCPT:HCPT是CPT的天然类似物,但HCPT不易溶于水、在中性PH下不稳定及对正常组织的不良反应极大限制了其临床应用。随着纳米科技的发展,HCPT的纳米形式表现出很高的性能,例如磁性纳米分子、纳米粒及水凝胶等。You等[34]利用微胶粒包裹HCPT在裸鼠动物模型中表现出缓慢释放,联合放疗具有放射增敏作用。另外,相关临床研究表明,HCPT联合放疗对呼吸、消化系统肿瘤有一定的增敏作用,其主要不良反应是骨髓抑制[35-36]。下一步,HCPT的主要研究重点在改造结构以降低不良反应并且与纳米技术结合,以解决部分物理特性的不足。
4.临床前试验化合物:①CPT417:一些研究者指出,CPT衍生物的毒性是由于其分子内内酯环的不稳定性[1]。还有学者在临床试验中发现,CPT类药物的主要缺点是不稳定的内酯环在体内快速水解导致药物活性降低,因此,稳定的内酯环延长药物活性成为研制新型CPT类药物的重要目标[37]。CPT417具有稳定的内酯环,在乳腺癌小鼠模型中显示出相比TPT更强的肿瘤抑制作用。同时还具有廉价、易合成及口服可吸收等优势[1]。② BMS-286309:有研究者将双氟加在CPT的核心基团上,增加其稳定性和生物利用度,BMS-286309在体外和体内实验中与CPT-11显示出了相似的抗肿瘤作用,但不良反应更低[19]。这些化合物的放射增敏作用均有待进一步研究。
二、TopoⅡ抑制剂20世纪80年代末,Evans等[3]通过研究依托泊苷、椭圆玫瑰树碱等协同射线对小鼠淋巴瘤L5178Y细胞系作用,发现TopoⅡ抑制剂能通过介导DNA双链断裂导致放射增敏,随着TopoⅡ抑制剂成为抗肿瘤化疗药物的研究热点,其放射增敏作用也逐渐被深入认识。TopoⅡ抑制剂按作用方式可分为2类:一类是通过稳定TopoⅡ介导的可裂解复合物(TopoⅡ-DNA)而杀死肿瘤细胞,称为TopoⅡ毒剂(TopoⅡ poison),例如蒽环类[多柔比星、氨柔比星(amrubicin, AMR)、柔红霉素、伊达比星][38]、依托泊苷(VP16)和阿霉素(ADM);另一类是通过抑制TopoⅡ的催化活性而达到抑制肿瘤的作用,称为TopoⅡ催化抑制剂(TopoⅡ inhibitor),这类药物包括阿柔比星(aclarubucin)、舒拉明(suramin)及喹诺酮衍生物伏利拉辛(voreloxin, SNS-595)等[39]。
相关研究认为,TopoⅡ抑制剂的放射增敏作用机制[40]与TopoⅠ抑制剂类似,主要包括:细胞周期阻滞,增加G2/M期细胞比例[41-42];通过稳定TopoⅡ和DNA之间的可裂解复合物,影响DNA修复[3]。
1.SNS-595:是一种二氮萘类似物,结构类似于喹诺酮类抗生素,通过嵌入DNA抑制TopoⅡ,导致S/M期细胞DNA损伤[43],但其没有蒽环类常见的心脏不良反应[44]。细胞实验及动物实验证实,SNS-595对乳腺癌、膀胱癌、胰腺癌、肠癌、卵巢癌、胃癌、肺癌等多种肿瘤具有杀伤作用[45]。Gordon等[43]研究显示,体外实验中,SNS-595对前列腺癌细胞株DU145、胰腺癌细胞株MiaPaCa-2及胶质母细胞瘤细胞株U251均有放射增敏作用,其中对U251增敏作用最强,SNS-595联合放疗能够减少U251接种的裸鼠肿瘤生长。在Gravina等[38]进行的一项针对大鼠模型的临床前研究中,SNS-595联合放疗组相比放疗组、SNS-595组、替莫唑胺组,显著提高无病生存率和总生存率。TopoⅡ抑制剂临床应用受限主要是因为P-糖蛋白介导的系统毒性和肿瘤耐药[44],SNS-595并非P蛋白药物泵的作用底物,能够不依赖P53介导细胞凋亡,因此避免了肿瘤耐药的常见机制[46]。SNS-595的药物不良反应及临床试验尚在研究中。
2.巴诺蒽醌(banoxantrone,AQ4N):AQ4N是N-氧化物的抗癌前体药,在细胞内基本没有毒性,但在乏氧的实体瘤组织和转移的组织内能通过生物降解产生抗癌作用,并能产生细胞毒性,是一种对乏氧肿瘤有抗癌作用的抗癌药[39]。AQ4N是细胞色素P450(CYP)的基片,AQ4N经历了CYP亚型的连续双电子还原,在一氧化氮合酶诱导下形成N-氧化物(AQ4M),然后进一步双电子还原生成AQ4,AQ4N不与细胞内DNA结合,但AQ4显示出强烈的DNA结合能力,并抑制拓扑异构酶。由于氧抑制了AQ4N的还原,在乏氧情况下,CYP能促进AQ4N的代谢,外源性的CYP也能增强AQ4N代谢。放疗或化疗杀死肿瘤细胞会导致乏氧细胞再氧合,AQ4通过抑制拓扑异构酶阻止这些细胞再生肿瘤复发[47]。AQ4N的体外实验显示,在乏氧情况下有很强的放射增敏作用,同时提高体内一氧化氮合酶的水平能够增强体内AQ4N的增敏作用[48]。Steward等[49]开展的一项关于AQ4N的Ⅰ期临床试验,22例食管癌患者给予静脉注射AQ4N联合放疗,18例疗效可评估,其中3例肿瘤体积缩小超过50%,9例病灶稳定,主要的不良反应是皮肤、尿液变蓝,淋巴细胞减少,白细胞减少,疲劳等。AQ4N的作用是提高乏氧实体瘤的治疗效果,也将对放疗耐受和化疗耐受的乏氧实体瘤提供更多的治疗机会。遗憾的是,目前AQ4N的临床试验研究处于停顿状态,但AQ4N类似物正在研发中[47]。
三、小结与展望放射治疗是多种恶性肿瘤局部控制的必要治疗方式,随着立体定向放疗等技术的进步,放疗越来越趋向于对靶区的精确照射,同时降低正常非靶区组织的照射剂量[50]。放射增敏剂能选择性地增强射线对肿瘤细胞的杀伤作用。本综述总结了近期Topo这类抗肿瘤药物作为放射增敏剂的研究进展及临床应用情况。尽管对Topo的认识有了很大的进展,且TopoⅠ抑制剂和TopoⅡ抑制剂中不乏临床应用广泛的抗肿瘤药,但总的来说,Topo抑制剂作为放疗增敏剂应用并不常见,主要受限于药物不良反应、不稳定性以及增敏效果的不确定性,且临床研究结果不尽人意,因此,有待进一步研究。目前TopoⅠ抑制剂新合成化合物的研究重点在于减低不良反应、增加组织稳定性等,其放疗增敏作用尚未见报道。TopoⅠ抑制剂下一步研究的重点在于通过化合物改构,新的结合形式(纳米等)以及Topo Ⅰ高表达患者筛选等方式降低药物不良反应,增强药物活性,并寻求个体化治疗可能。TopoⅡ抑制剂更多的研究工作可能在于其临床试验研究及新化合物的合成开发,以得到肿瘤特异性更高、增敏作用力更强、不良反应更少的Topo抑制剂,为肿瘤综合治疗提供更合理有效的可能方案。
